How do I generate CQM data?

Quality Measures statistics can only be generated through the new Certified Electronic Health Record Technology dashboard tool, Quality Analysis and Reporting for Centriq and Thrive.

Where did Evident get the questions and content for the new documentation areas?

Evident development staff formatted the questions and sections to match requirements set forth by CMS as part of the specifications. For questions regarding the logic behind the documentation required, sites should refer to the specifications published on the USHIK website.

Which pieces of documentation should I address for my patients? Should I address all questions?

Familiarize your staff with the measures your facility is trying to meet, and familiarize yourself with the parts of the content that correspond with those measures. Evident Documentation support can assist with making those questions required and hiding questions that are not needed.

How do I access CQM documentation (Inpatient)?

Three methods are available to access the CQM documentation. First, it can be accessed through the Clinical Quality Measures document in the Clinical Documentation application. Second, it can be accessed through the CQM Documentation launch point in the Point of Care Virtual Chart. Last, the CQM documentation can be accessed through Physician Documentation by setting up Quality Measures sections and adding those sections to a document. Please refer to the online presentation for Structured Documentation for Clinical Quality Measures for details on the setup and pathways for all three methods of accessing CQM documentation.

Can flowchart widgets still be used?

Yes. Some questions no longer apply as some measures have been retired. We have no plans to enhance the functionality of the widgets as we move toward using the CQM content.

What security setup will my staff need to allow them access to the CQM documentation?

Staff members who will be entering documentation using the new CQM content will need to have the application Documentation added and marked as active.

How does my patient data get to QARCT?

Patient data is pushed nightly through an automated cron job.

How do I know the refresh is working?

When the refresh is complete, profiles will show in data sync. Future development will allow for a message to know the refresh is running. When the Refresh is triggered, these profiles are sent to the dashboard in one large piece. They are then inserted into the database, and then a Recalculation is triggered. This can be verified by checking Data Sync.

What securities do I need to be able to use this new tool?

Behavior Controls will need to be set up in order for the necessary users to access the reporting tool. There are ten different behavior controls to choose from in Identity Management under the Quality Measures application, though not all employees accessing the dashboard will necessarily need each dashboard option:

  • Allow Access to View Dashboards
  • Allow Ability to Modify General Settings
  • Allow Access to ‘Clear Data From All Dashboards’ in Refresh
  • Allow Access to ‘Repopulate All Dashboards’ in Refresh
  • Allow Access to Event Log
  • Allow Ability to Delete an Attestation
  • Allow QRDA Cat I Measurewise Generation *We do not recommend that sites use this option. This was required for certification and cannot be used for submission. It restricts the data in a QRDA I down to just the data for one measure. Sites need to report all of the data on a patient for all of the measures they are submitting.
  • Allow Ability to Delete a Dashboard
  • Allow Ability to Create New Dashboards
  • Allow Ability to Edit Dashboard Settings
I have added all of the necessary Quality Measures behaviors, but I am still unable to access the QARCT. What could be causing this?

Users should confirm that they have added all appropriate behavior controls in Identity Management and that these are set to Allow.

When I select the Populate Dashboard icon, why does the “In progress” swirl next to the name of the dashboard continue to spin?

This occurs because there is nothing updating the screen. You can click on something else and come back, or just go to the dashboard and wait for the Last Updated time to change.

What modifications can be made to CQM documentation (Inpatient)?

Within Clinical Documentation, facilities can choose to hide any questions that they will not be using. Questions can also be set as required so that the user is prompted to address the question before proceeding. Reflexes can also be set up for additional documentation, orders, and charges. Within Physician Documentation, CQM Sections can be added to any existing Physician Documentation templates, and you can customize how these sections are labeled. The specific wording of the questions was created based on CQM specifications. We welcome feedback from facilities on the CQM content. For some of the measures, why do the visit numbers and the discharge date and time not always display?

Measures such as 56, 66, and 158 do not require an actual encounter to make it to the initial patient population. Items such as visit number and discharge date and time will not display, and this is the correct functionality.

How do I export data from QARCT into an Excel document?

There is an Export option on the Measure Detail screen that will allow sites to export the list of patients meeting the specific populations.

How do I sort and search for data within QARCT?

Most columns in the Measure Detail screen, Manage Dashboards screen, and the Event Log are sortable. Select the name of the column to sort. The search field will allow you to search by anything within the Measure Detail screen. After typing in the search field, press enter to trigger the search.

How do I delete a dashboard?

Dashboards can only be deleted if they have been cleared of data. This is done by selecting Reset for that one dashboard. Reset does not delete the data, and it is still stored in the MongoDB database. Once the dashboard has been cleared of data, it can be deleted by selecting the delete icon from the Manage Dashboards screen.

Is there a way to create a favorites list of dashboards?

The speedometer icon will display your favorite dashboards. You can choose your favorite dashboards by selecting the star icon on the Manage Dashboards screen. From within that list, you can select the Home icon. Selecting the Home icon for a dashboard will allow your screen to default to that dashboard when you access QARCT. This setup is optional, and not setting a home dashboard will direct you to the Manage Dashboards screen. The facility ID is included in the token, so you will have different lists that display depending on which facility you are in when accessing QARCT.
